Terms and Conditions

This web site (“Site”) and related services are provided by Summit Health to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this Site.


Your privacy is of the utmost importance to us. Summit Health will use your confidential medical information in order to provide you health care services. Summit Health will, at all times, maintain the privacy of your confidential medical information and will not disclose medical information about you unless you authorize the disclosure of your information or the disclosure of the information is permitted by law. Please review Summit Health Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyChart that contain medical information are subject to state and federal laws governing the security and confidentiality of medical records.

II. MyChart Features

A. Electronic Messaging

MyChart should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is three business days. If you believe you are experiencing an urgent or life threatening health condition, please call your physician’s office, go to an emergency room of a local hospital or dial 911.

If your provider is out of the office or unavailable, messages sent via MyChart may be routed to other authorized caregivers within Summit Health in order to facilitate a timely response to your request.

Summit Health may send you messages via MyChart. It is your responsibility to monitor these messages and respond in an appropriate manner.

B. Secure Communications

All communications between you and Summit Health using MyChart occur over a secure connection. You are required to provide Summit Health providers a valid e-mail address and it is your responsibility to keep this address updated if you change your e-mail account. You will receive Internet e-mail messages notifying you that new information is available in your MyChart account. Although these e-mail messages will not contain any confidential medical information, the notification that new information is available by accessing MyChart may be information that you would not want others to know. Please take this into account when providing an e-mail address or using MyChart. All of the confidential medical information available to you in MyChart is protected and securely maintained by Summit Health.

C. Activation Code

When you initially enroll in MyChart, you will be provided with an activation code. The activation code you receive will give you the opportunity to create a personal login name (called MyChart ID) and a password. Once this activation code has been used it will no longer be valid.

It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that its security has been compromised. You may change your password at any time.

D. Features

At this time, Summit Health anticipates that the MyChart web portal will provide the following services:

  • Ability to review, request or cancel your appointments
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information
  • Ability to receive selected lab results
  • Ability to communicate with your health care provider via secure messaging regarding health concerns
  • Ability to request prescription refills
  • Ability to link to other sites that will provide you information

IV. Termination of Access

Summit Health Providers may restrict or terminate access to MyChart at any time. If your use of MyChart violates these Terms and Conditions, Health Care Providers may permanently terminate your access to MyChart.

V. Restrictions on Use and Ownership

The content on the Site is provided for Summit Health and its patients only. All other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Summit Health or the third party that may own the trademark, service mark, or logo.

VI. Accuracy and Integrity of Information

Although Summit Health attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy is identified, you should inform Summit Health so that it can be corrected. Information contained on the Site may be changed or updated without notice.

VII. Availability

MyChart may not always be available to you due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and you are not guaranteed access at any particular time.

VIII. Use of Cookies

"Cookies" are small bits of information generated by our web server and stored on your computer's hard drive. Cookies can "remember" what information a user accesses on a web page to simplify subsequent visits by the same user.

IX. Confidentiality

Both Federal and Oregon laws govern the confidentiality of patient health care records. Summit Health is dedicated to keeping your medical information confidential. Our employees are subject to strict standards for maintaining the confidentiality of your medical information. By allowing you access to your information, you must share in this responsibility.

X. Security

The Site places considerable emphasis on the privacy of medical information and compliance with State and Federal privacy laws. MyChart is configured to be secure from unauthorized access, but Summit Health and the Site cannot be held responsible for:

  • Absolute security of all electronic communication transmissions between patient and Summit Health
  • Unauthorized disclosure resulting from a user not logging out of an active session;
  • Unauthorized disclosure resulting from a lost or stolen user ID and password;
  • Unauthorized disclosure resulting from information printed by the user from MyChart;
  • Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
  • Similar events outside the substantial control of Summit Health or the Site.

XI. Links to Other Sites

Summit Health may create links on the Site to other web sites that we think may be of interest to you or provide further information. Summit Health makes no representations whatsoever about any other web site that you may access through this Site or any entity that may post its advertisements on this Site. When you access a non-Summit Health site, it is independent from Summit Health and Summit Health has no control over the content on that web site. Summit Health does not endorse or accept any responsibility for the content or use of any linked site. If you decide to access any third party web site linked to this Site, you do so entirely at your own risk.

Summit Health has not reviewed the privacy policies of any web sites linked to or from the Site, and you should exercise care when visiting any other web site. We would like to hear from you if you find any web sites linked to or from the Site objectionable.

XII. Your Right To Object

If you believe Summit Health(s) is using your personal identity information in a way that is not consistent with the Terms and Conditions, you should contact us at once.
